-Does my plan require pre-authorization for psychotherapy (written approval from a primary care provider)?

-Does my plan limit how many therapy sessions per calendar year I can have? If so, how many?

$150 for our intake session and $130 per routine 55-minute session

I am happy to provide you with a superbill, which contains all the information necessary to file a claim with insurance companies. Many insurance companies will reimburse 50-80% for out-of-network providers.
